Occupational Therapy Assessments
An Occupational Therapist's area of expertise is assessing function. Our Occupational Therapists can assess an individual's functional abilities in their home, work or school setting and analyze the impact of an injury or disability on their ability to function in their activities of daily life. We can provide recommendations for treatment, assistive devices, environmental modifications, or support services that are specific to the client's unique needs.
Future Cost of Care Assessments/Life Care Plans
In cases where an injury is permanent or catastophic, a Future Cost of Care Analysis or Life Care Plan can be prepared. A Future Cost of Care Analysis or Life Care Plan is a dynamic document based upon published standards of practice, comprehensivie assessment, data analysis and research, and provides an organized, concise plan for current and future needs with associated costs, for individuals who have experienced catastophic injury or who have chronic health care needs. Capacity Assessments
A Capacity Assessment can be completed to determine whether a person is capable of making decisions regarding their personal care or finances (property). Capacity Assessments are typically completed when a person does not have a Power or Attorney. If a person does have a Power of Attorney that does not state how incapacity is to be proven, the opinion of a Designated Capacity Assessor's opinion may be required.
If a person is not mentally capable of making decisions with respect to their personal care or property, a separate person can be given the legal authority to make these decisions on their behalf, and becomes their authorized substitute decision maker. This process is known as "guardianship".
Under the Substitute Decisions Act in Ontario, a Designated Capacity Assessor is the only legal authority that can complete Capacity Assessments for the purpose of assigning guardianship of personal care and property. These assessments are completed according to strict SDA guidelines, and can be done without court action. Designated Capacity Assessors have completed a training and certification program through the Ministry of the Attorney General, are members in good standing with their professional college, and are required to complete ongoing education offered by the Minsitry of the Attorney General.
We offer Capacity Assessments under SDA guidlines as follows:
Issues of capacity outside of the scope of the SDA are addressed in capacity letters of opinion.
